We provide company credit cards to certain officer - these officers are to use their cards for company purposes, and when they receive their statement, they are to complete a credit card expense report, attach their receipts and submit to their manager and ultimately to A/P.

Does anyone currently have a good template of a company credit card acknowledgement?
