I've seen this addressed before, but really want to make sure that I'm clear on this.

Our customer ABC has online banking with ACH capabilities. ABC has given login priveledges/access to their accountant (ABC owner signed giving these rights). Last week 3 ACH credit transactions were initiated using the customers access, resulting in a $90,000+ loss to the customer ($10,000 has been recovered at this point). So even though the bank is not at a loss, there is no known customer, etc... we still need to file a SAR correct? And just provide as much information and detail as necessary? The PD and Secret Service are involved and it was also "supposedly" taken to the US Attorneys office....

We've never had a case like this before and I just want to make sure that I cover every possible base.

Yes, it exceeds the SAR thresholds, you need to file.

If these were credits how would there be a loss to the customer?

An ACH credit transaction credits a third party and debits the client's account at the bank.
