Our Adverse Action Notices have a spot for the applicants Social Security Number. Is there a requirment to have that on an adverse action notice for Reg. B or FCRA? Mailing out a form with an applicants name, address and SSN is just asking for ID theft. Does everyone still list SSN on Loan Documents?

No. It is not a requirement. Are you working with ARTA? I've worked w/ ARTA before and there used to be a spot for the SSN.

Unless it is a state law NO!

SS# is not on any of the model forms, and is only an invite to ID theft as you mentioned.

No not ARTA, but I'm sure it's one of those things that has always just been on the form.
