A few months ago as I was drafting our CIP procedure I located a "Cheat Sheet" of sorts that had two columns, "You are required to verify identity of," and "You are not required to verify identity of." I am not able to locate the source document. Is anyone familiar with this document?

Been there, done that. I also searched the threads but didn't see anything. It is formatted with two columns; one requiring verification and the other not.
