Are two different formats required for the periodic statements, or can the full statement with all of the delinquency boxes be used for all. Leaving the delinquency information blank on current loans.

There should be no problem with using a pre-printed form with spaces for all the potential required information, sections of which would be left blank (or, if you prefer, with a "None" or "Not applicable" entry).

On the other hand, dynamic forms printed "on the fly" to conform to the data to be included can avoid problems with information that exceeds the size of a pre-printed form's allotted boxes.

I think either approach would be acceptable.
