Wanted to check and see if anyone had a guest injury procedure/form they would be willing to share before I recreated the wheel....

It should be part of your Illness and Injury Prevention Program (IIPP) which is probably administered by HR.

We do have one there, but it is from our Worker's Comp provider ....
My insurer has a sample form that we can use if we want to or don't have one of our own - I may end up going that route at some point - However, their form is extremely onerous....plus there are no accompanying procedures.....
