When completing this form for a multiple party account,are all owners of the account required to initial in the related areas? Or is one set of initials sufficient, such as the primary account holder?

I am having everyone initial (except, of course, POD). Being the reason for the for is to assist in defining ownership both during life and after death.

I don't have a statutory cite for my reasoning however.

That is how I was leaning as well. Also what if changes are requested after the accuont is opened? Can one account holder make the change or will it require all parties to verify the change?

Confirming this form is used for consumer accounts only and not for business accounts?
