My Audit department was given the task to do a Management's Audit. This high level audit require us to develop a questionnaires for the senior level management i.e. General Managers (GMs). Can any one help me with these type of questionnaires, any one has one or tell me where to find. I appreciate your help.

I'm not familiar with a "management's audit". Is it anything like a director's exam? What is the objective?

The objective is to ensure that the General Managers (GMs) are aware of the risks in their areas and whether these risks are being addressed. In addition, the Managing Director wants to be aware of how those GM are conducting their jobs.
