Does the Internal Audit Charter need to be approved by the Audit Committee on annual basis?

I have the Audit Committee review and approve it annually.

I would say yes.

I don't have an Internal Audit Charter.

I have an Audit Committee Charter and an Internal Audit policy, both are reviewed and approved by the audit committee each year prior going to the full board with all other policies for approval.
