Just wondering what others opinions for this might be. When completing a SAR, if there is an unknown subject would you leave item 21 (relationship to financial institution) blank or would you select no relationship?

I have previously selected no relationship to institution, but I make no claims about the correctness of any of my decisions.

If you indicated that "all critical information is unavailable" that suggests to me that you are not in a position to know whether the person does or does not have a relationship with your bank...I'd leave it blank or call the Resource Center and see what they say.
