Do escheatable cashier's checks get reported by the name of remitter or who it is payable to?

This may be state-specific and you should check the instructions for filing. In New Mexico they tell us to list the names and addresses of both the remitter and the payee, specifying each. Since in some/most cases you won't know the payee's address you would put 'unknown', but we certainly have the address of the remitter as we sell only to customers.

To answer before this gets moved, most states that I have seen will require you to list both the payee and remitter, and identify them as such. They should also have due diligence instructions for who to send notice to regarding any upcoming escheat of the funds. If you post which state this is for I can take a quick look to see if I have anything that will clarify what to do for that state in particular.
