Can someone point me to the latest model statement for HELOCs? I'm trying to determine whether a loan which has a “Balance Subject to Interest Rate”, should display the different period(s), rate(s) and balance(s) that it was subject to.

I know that the proposed model statement forms for HELOCs by FRB got shelved, but is there any other templates available?
