I know that we report purchase loans in HMDA but what about those that were modified do we still report them and ignore that they were modified.

You purchased a loan that had been modified? You report the loan as purchased assuming it otherwise meets HMDA reportability. You are purchasing the loan, regardless of any changes due to modification.
