Does the Privacy Notice have to be on 8 1/2 X 11 size paper? I thought a while back that we could not include it in our disclosure booklet because the vendor could not meet the size requirements. If we include it in our disclosure booklet on two pages with the required font size, is that all that is needed?

The GLBA Model Form requirements do not stipulate 8 1/2 x 11, just need to adhere to the orientation, format and font size requirements. This will keep you within the ballpark of 8 1/2 x 11, but can go slightly smaller. Also consider having a larger 16pt ADA version available upon request.
