That form isn't required anymore. The information should be included on your Loan Estimate.

Unless you're doing an AAN, then it is still required. Since you're probably ordering an appraisal on a turn down and all... smile

For TRID loans, true, but for non-TRID loans secured by a first lien on a dwelling, you still need the stand-alone appraisal notice. (Referencing TR's post.....since i didn't get in right behind her.)
