You can do quizzes in some free survey products (most have a free level) like Survey Monkey or Typeform. I recently heard that Google Docs also now has a quiz feature but I have not tried that.
There are some paid (but not too expensive) quiz products such as ProProf.

Thanks for the ideas. I'm trying to use Typeform. It says you can identify the respondents by building in a place their name, email, etc. in the Typeform. Do you know how to do this?
You just add text fields. Call one Name. It will add a space for users to enter their name. You can add an email field, etc. Their help section is pretty good.
