Our flood determination notice has a space for the lender's signature/date. Is the lender required to sign the notice that goes to the customer?

You need to date the determination, but there's no requirement to have the lender sign it. And I wouldn't do it. If you start this practice and then miss it on 1, it looks out of the ordinary or a shift from your procedures. I would question why your SFHDF has a space for the lender signature.
