Our marketing department is looking to send a survey to our shareholders. For every survey completed, the individual will receive a $10 gift card. Is there required disclosures? Are there any implications with doing this?

As you are giving the gift card to everybody that completes the survey, I don't think this would be considered a sweepstakes or raffle, which could be restricted or prohibited depending on State law.

Survey to existing customers with the chance to win a $50.00 cash reward. I'm thinking this is considered a raffle and not be allowed but can someone point me to something more concrete than my memory

Having to complete a survey for a chance to win is "consideration", so it makes it a lottery. If it is open to anyone, then it is not.
