On our stop payment forms there is a place that says - VERBAL STOP PAYMEBNT ORDER REQUEST ACCEPTED ON ___________ BY ______________________________

If we have someone call in, and give a verbal request to do a stop payment, do they have to still come in and sign the form?

Look at your state's version of the UCC. If it provides for verbal stop payments, it will generally say that, unless it is confirmed in writing, verbal stop payments are only good for XX days. After that, the check is properly payable.
