We don't offer any type of Credit Insurance. I wish our forms provider offered a Note that didn't include the Insurance disclosures, but they don't.

Therefore, on the Note, when you don't offer any insurance, is it best practice to leave the Insurance section completely blank and not have the customer sign or is it best to mark the "None" boxes and have the customer sign?

I would never advise leaving anything blank on a note, and would recommend completing any non-applicable blanks or sections with 'NA'.

Check your state laws, some may actually address this issue.

Just reviewing an old exam, Wisconsin has such a prohibition on blank writings for certain fields.
