Can anyone share instructions for completing this form. Last minute was assigned to complete it and I'm lost. Please help!

To add to my previous post, we have zero instructions and this form is new to me. It asks for "total volume?" Does that mean total deposits or should I include loan balances too?

I believe it would be total deposits. Loan products are asked about separately and are more limited in the form.

There is a complete instruction document on the OCC's Banknet system. If you have been granted the MLR completion, you will need access to BankNet to submit the form. All the necessary resources should be there. Hope that helps.

Please find a copy of last year's for the sake of consistency and follow the Frog Lady's advice. Only financial institutions supervised by the OCC have the great joy of completing this form. Most of those assembled here have no idea what you are talking about...
