What's the worst case scenario if I didn't get a SAR reported at the next board meeting?
Any suggestions for me after the fact?

Get it to them the next meeting and pray that the good lord doesn't strike you down with a lightening bolt. smile

As long as it is not a pattern or practice, I don't see it as a show stopper. Errors do occasionally occur.
