What would be the flood requirement on a condo unit that has "first level parking" then the 1 level of condos is above the parking area? Would a survey be needed to show the actual units are out of a flood zone?

The condo building is in either a SFHA area or it is not. If it is, a condo unit on the first floor or the 30th floor needs flood insurance. Parking garages do not impact the building location.
