Individual opened a checking account. She provided an unexpired Non-US Citizen Driver License and Visa (does not expire until 2013). She signed the w-9 on the signature card and the first two boxes are checked (taxpayer id number box and not subject to backup withholding box). On the TIN line on the w-9 an 8 digit number from the bottom of the Visa is recorded.

In trying to determine if handled properly:
1-Should the w-9 have been completed?
2-Should a W-8BEN have been completed instead of w-9?
3-Should the passport number have been recorded anywhere?

It could be that she has passed the "substantial presence test" even though she is not a US citizen or green card holder. It is really up to her to decide which form she needs to complete.
