Are we required to use the form in this exact layout or can it be reproduced in another format? Our operations would like to have it included with our notices of a past due payment, but the formatting is different.

Typically, forms with an OMB Approval # cannot be altered without specific provisions that allow alteration. I am not aware of any for this form.

While I completely agree with Randy, I have advocated changes in what bankers sent but the only modifications were required customizations for the bank and blasted typo corrections that HUD just couldn't do. I'd never tell a bank look dumb with blatant typos.

I think you could add a notice to the beginning or end of this, but I wouldn't get carried away.

In attempting to verify we had the link to the Legal Assistance Office locator correct (http://legalassistance.law.af.mil/content/locator.php) and the link goes to a "not found" page. If we remove everything after .mil it works. As Randy said, we don't want to mess with OMB forms but I was wondering if anyone had heard anything about a revised notice? HUDs website still has the current version with the broken link.
