If a customer calls our contact center and requests a copy of their statement sent via email, do we have to go through the e-statement consent process? These customers have already received a paper statement but have "misplaced" or "lost" it and are requesting a one time copy of the statement.

As long as you are comfortable with sending customer statements through an unencrypted system (unless you have an encrypted e-mail server that you use), no - it would not be subject to E-Sign as it is not replacing the actual paper version that is delivered monthly.
