Individual comes into the bank to cash a paycheck for $15,000. The check is drawn on the ABC Business. It is an On-Us check…does the bank need to indicate the name of the business in Part 1?

Unless you have reason to believe that the transaction was being conducted on behalf of the business you would not include the business in Part 1. The individual would be conducting on their own behalf if it is their payroll check and would be who you list in Part 1. The account number of the business would be listed in Item 22.
