Is there any reason that multiple branch contacts couldn't be listed on Adverse Action Notices? I can't find any information that only one can be included. Additionally, does a written adverse action notice require the signature of an officer?

You don't even have to list a specific person, let alone sign them.

(2) Content of notification when adverse action is taken. A notification given to an applicant when adverse action is taken shall be in writing and shall contain a statement of the action taken; the name and address of the creditor; a statement of the provisions of section 701(a) of the Act; the name and address of the Federal agency that administers compliance with respect to the creditor;

A phone number is only required if you don't provide them the specific denial reasons.
