If our bank approves a closed-end, fixed rate consumer loan that will be secured by a 2nd mortgage lien on a principal dwelling, do we need to provide the Protect My Kentucky Home notification? If yes, must this form be signed to acknowledge receipt or may we just mail it out before closing and document the loan file as to the date sent?

Yes - a 2nd lien is covered. Since the State's form says "Please Read and Sign" and instructs the lender to give a copy to the homeowner, I will let you be the judge on the rest of it.
