Couple of ways to look at it. Did the customer refuse to provide the information OR did it just not get asked? If the customer flat out declined to provide it I would consider filing a SAR.
It's not a critical field - but denying to provide information requested is definitely a red flag.

Not enough facts that appear suspicious, but the decision is up to you. Many customers might not want to provide phone number and might not understand reason that bank needs to know.
