We ended up making the loan to the original applicant, using the same app with a notation re: the co-applicant's removal. We collected GMI on the co-applicant, which the lender left in the file. We do not report GMI for the co-applicant, correct? Should I just remove it from the file?

No, don't remove it from the file (and you are correct not to report the GMI). It was properly gathered at the time of the application, and (assuming you have properly documented the changes that happened between application and closing) having it in there is, IMO, appropriate and NOT having it there, when it's obvious the application began as a joint venture, would appear to be a violation.
