Welcome to BOL. That is a pretty broad request. If you are looking for ideas, there are some on the web, for example: https://www.epcor.org/docs/samples/ACHPolicy-TOC.pdf

Most banks are going to be hesitant to share proprietary bank policies with a third party. Likely they would have to get board approval to do so.

Often times your state or regional ACH payment association will also provide ACH risk assessment and policy templates or direction for you to utilize as well.
