If you issue new debit cards orally, what kind of procedures do you do to ensure you are issuing to the customer that is authorized?

Reg E permits the issuance of access devices based on an oral request. That being said, while you are polling other banks, you may also want to check with legal counsel to see if there are any concerns issuing a card based on an oral request if you have no written evidence that the customer has agreed to the card terms and conditions.
