Do you have a model form of what you use for pre-application worksheets prior to receiving a full TRID application?

There is really no model format for a pre-application worksheet in its entirety, but H-26 of Regulation Z has the model statement that must appear in at least 12-point font. You can't use anything that resembles a Loan Estimate or Closing Disclosure, either.
