A customer requests a change in the payment due date on a mortgage loan, i.e.from the 10th of the month to the 30th of the month, The bank completes a change in terms document to change the due date as well as the maturity date to reflect the change in due date. Would this practice be a MIRE event for Flood?

Yes it would, but most banks would not extend the due date of the loan for that sort of modification to avoid having to file a modification to the deed of trust or mortgage.
