Currently our Bank requires a signed Debit Card application for initial issuance.

Does your Bank have a Debit Card application requirement for initial issuance?

I understand a signed (written) Debit Card application is NOT required under Regulation E - that an oral request for a Debit Card is acceptable - the written application is obtained for "proof" the consumer requested the Debit Card. [u][/u]

We initially required applications and had lenders reviewing them as missteps could lead to a credit situation. After many long nights and lost weekends management realized we weren't denying anyone. And our track record indicated the risk was there, but was minimal. The paperwork didn't help anything and wasn't updated by the customer if there was a change. So there were lots of costs, little benefit from them. YMMV and it may depend on yur volume as well.
