For prepaid cards we have an issued date and an activation date. It’s not unusual for cardholders to sit on a card for months before activating it. We are trying to come to an agreement on what is a “new account” for Reg. E? Does the clock start at card issuance or on card activation?

New account for what context? If you are talking error resolution timing, it's based on 30 days after the first deposit.
1005.11(c)(3)(i) The applicable time is 20 business days in place of 10 business days under paragraphs (c)(1) and (2) of this section if the notice of error involves an electronic fund transfer to or from the account within 30 days after the first deposit to the account was made.
