Am I correct that joint intent must be established for each application? For example, if we have high volume business borrower who indicated that they will be include an additional guarantor on each of their loans would it be correct that for each new request we still need to ask if who they intend to be on the loan?

Do you not require a guarantor by policy? If so, there is no intent, it is a loan requirement.

One of them is just the spouse of the principal of the business so we would not be able to require them to guarantee.

So the spouse controls enough personal assets individually to make them a viable guarantor and you require a guarantor on the loans? If you require a guarantor and allow that to be someone other than a principal in the business, then yes, you would need intent on each loan.
