Just wondering if our bank could include the name and address of the Credit Reporting Bureau on the adverse action form - even if we don't deny due to the credit report. The "box" would not be checked, but the name and address would be included.

If the box isn't marked, meaning the name and address of a bureau is not applicable, then you should be fine. I recall years ago we used to have this preprinted on the forms as we only used that one credit bureau.
