Wondering if any of you use an audit survey after the completion of an internal audit? I would like to get feedback from the managers on the audit process. Would you be willing to share your audit survey?

I utilize an audit survey and find them very effective. PM me and I can send you a copy.

I use one as well (I think I found the format on Audit Net) but don't get a great response - about 50%.

I also send out an assessment survey at the conclusion of each engagement. We've had a good return rate and they have proven to be beneficial. Granted, the assessment is often reflective of the audit rating, but they still provide important insight to the perception of Internal Audit and its effectiveness.

One way that we do surveys is verbally..... We just had a managers meeting that was held with all of the branch managers and the CEO - they allowed people to "vent" their frustrations openly which proved to be very effective - of course, the department personnel were not in attendance which allowed the branch managers to openly vent who they were pleased and displeased with and why - I think it was a good way to allow people to say what they wanted and have upper management follow up afterwards.
