Is there a form that may be purchased or a separate signature card, or just an additional form that you may have created yourself that is given to employees concerning waived maintenance fees on checking accounts?
I believe we have it in our employee handbook as a benefit.

Ours too. It is in the employee handbook. When the account is opened on the system it is specifically coded as an employee account which means no fees.
