Does anyone have a wire transfer agreement that they would be willing to share?

When you say wire transfer agreement do you mean the form that the customer signs requesting a transfer each time or the form that the bank keeps on file for future phone, fax, mail or email request that contains security information etc.?

I am talking about the form that we keep on file for future wires.

We use a Banker Systems form, you could probably obtain a sample from them.
