Does anyone have a disclosure for a tiered rate account that they would be willing to share? I have already pulled and provided a copy of the sample forms in the Appendix to Reg DD, but would like to provide a real life form to supplement.

We have one, but we use the model language, so ours is just like the samples you've already provided.
