Hope someone can help regarding this form. We have very little info - it appears that it was applicable as of 010104. It involves $ amount of YSP and itemization of points, fees and commissions. A signed copy must be retained and a completed hi-cost loan worksheet. Does anyone know about this state issue or can suggest where more info could be found? Thanks!
