The only disclosures that I can find for a Commercial Bank on credit life and disability insurance is to disclose that the customer has a right to get the insurance elsewhere and to supply the policy within 30 days with application for insurance policy given at closing. Did I miss anything?

The Ohio Dept. of Insurance issued a reg back in 1983 (more or less) that requires a separate disclosure be given to customers with specified language about how credit insurance is optional, etc. This notice must be initialed by all customers to indicate whether they want or do not want the coverage.

As far as I know, it has not been repealed. I still see the disclosures in loan files when I do reviews for Ohio clients.
