The Ohio Dept. of Insurance issued a reg back in 1983 (more or less) that requires a separate disclosure be given to customers with specified language about how credit insurance is optional, etc. This notice must be initialed by all customers to indicate whether they want or do not want the coverage.
As far as I know, it has not been repealed. I still see the disclosures in loan files when I do reviews for Ohio clients.
