Does anyone have IT standards and guidelines as they would relate to work performed for maintenance and system changes.
It would be interesting to know how you are handling this issue - log every change and make sure every change is documented...log only critical field changes...ensure source documents to make changes are initialled, etc. etc.

P.S. If you have a source document to refer me to for this guidance, that would be appreciated also!

It sounds as if you are talking about the risk management process called Change Management or Change Control for IT systems. Change Management, the process, involves the tracking of all system/field changes to IT production systems (i.e. those used regularly for banking processes) to ensure no unneccessary risks are being taken.
