we use an Affidavit and Indemnity Agreement for Lost Instrument/Certificate of Deposit. Our form is outdated and wondering if someone knows if there is a version newer than 1990 and where we can get it?

We use one that was drafted by our attorney. However your software company, if you use one for issuing deposit accounts, should be able to help you. If not contact your attorney or the IBA may be able to help.
