Is the Identity theft affidavit something that the consumer completes when they notify law enforcement that their identity has been stolen, or is this a form that I should have and get the customer to complete?

The customer completes and submits it to banks, creditors etc. The affidavit can be found on the ftc's website: www.consumer.gov/idtheft

There is also a free brochure that you can order from the FTCs website called Take Charge: Fighting Back Against Identity Theft. I ordered a bunch and sent them to all branches for customers and employees.
