Did the loan close within 3 days or was it denied within 3 days? If so, no violation. Other than that, you did not give a disclosure that was required at application (or within 3 days) sorry, you can't fix it.

Generally, examiners like to see that your processes to detect problems are working and that you are taking steps to correct going forward.
