We have some applications taken by telephone that the applicant responded to Ethnicity as Hispanic, sex as female, but did not want to choose a race. (I'm sure it's that same old problem that they felt none of the categories applied.) We have used code 6 - information not provided by applicant in telephone application. Our system has not kicked it out as a validity or any other kind of error. Is this the correct way to report this? I guess the real question is, if you report "information not provided by telephone" in one reporting field, does the same have to be reported for all of the monitoring categories on the LAR, or can some be completed and some say not provided?

You are fine. It is acceptable to have some completed and some say "not provided". That's really what happened and the regulation allows for it.
